I reminded me of the season finale of house. i go in and here is my zeb with a red rash on him and his eyes bugging out of his head dead. I noticed the rash this morning and treated with melafix but I am hoping that none of the others get it. Any ideas?

new setup with established sponge filters.
I guessed it was a bacteria but I have never seen the eyes pop out before any ideas?
Eyes popped out of his head!

Nitrite level 3ppm causes major stress on any fish. You should make a huge water change every day until level is 0, or move your fish in matured tank. The zebra died (or got sick) propably mainly on stress related disease, in this case maybe bacteria, maybe even TB, wich is common amongst aquarium fish. If the other fish seems "healthy" you shouldn't use medication, just change water to keep nitrite level as low as possible. Try to feed your fish as little as possible, until the tank has matured and nitrite level stays on 0ppm.
So the mistake you have done, is to put fish on unmatured tank, where is lack of good bacteria, which disingrates the ammonia to nitrite and then to nitrate. If you have a matured tank, or your friend does, take a half quarter of a gallon (5 desilitres) of bottom sand from it and move it in your tank. This should help achieving better bacterial balance.

You can also add established filter media from another tank. BioSpira could help, also. Add a teaspoon of salt per gallon of water to help combat the poisoning they get from nitrites. It affects their bloods ability to carry oxygen. Sorry to hear of your loss but you will definitely lose more if you don't get your water parameters under control in a hurry. Stop feeding at all until you have 0 ammonia and 0 nitrites, IMO.
